Booking

To book Dr. Yang as your anesthesiologist, please do the following:

1. Fill out and submit the online forms:
   • Medical history
   • Informed consent
   • Financial information
   • Other instructions
2. Confirm and communicate with your dentist a date and time that works for you, him/her, and Dr. Yang. Your dentist has access to Dr. Yang’s availability and can help arrange the appointment.
3. Notify Dr. Yang of the date and time selected by you and your dentist.
